Registration and Check-In Procedures

Registration for the 2009 Midwest Soccer Challenge will take place at the following location, dates, and times:

Date/Time/Location
Friday, April 17th, 2009 12:00pm to 9:00pm - Hazelwood Park
Saturday, April 18th, 2009 6:30am to 12:00pm - Hazelwood Park

We encourage all teams to register on Friday if possible. Registration will be at the Headquarters tent at Hazelwood Park. All registration must be completed at least ½ hour before game time of your first game.
All items below must be provided prior to or at registration:

USYSA Teams
* USYSA Player Passes
* Medical waiver forms must be shown, however, they will be retained by your team & kept field side.
* Travel Papers (if out of state team)
* Roster (word document provided)
* Guest player form as required by USYSA state associations (PDF document provided)

US Club Soccer Teams
* US Club Soccer player cards.
* Official US Club Soccer roster is required (and all guest players must be shown on the official roster).
* Medical waiver forms must be shown at registration. However, they will be retained by your team & kept field side.
* Copies of insurance cards for all players must be provided at check-in
